REQUIREMENTS FOR DUPLICATE REGISTRATION :-

- Application on letterhead or Rubber Stamp With Name & Registration Number.
- Police Complaint in Original.
- Affidavit on Stamp Paper of Rs. 100/- mentioning how the practitioner has lost
the registration certificate & the practitioner has not applied for duplicate
registration earlier.
- Two passport size photographs.
- Fees of Rs. 2000/-

REQUIREMENTS FOR CHANGE OF NAME :- |
- Application on letterhead or Rubber Stamp with Name & Registration Number.
- Marriage Certificate in Original with Xerox.
- If there is change in the first name also practitioner has to submit Govt.
Gazette in Original with Xerox of the first page of gazette & the Xerox copy of
the page on which Change name has appeared.
- Fees of Rs. 500/-

REQUIREMENTS FOR PROVISIONAL REGISTRATION :- |
|
Applications for provisional registration should be submitted by college
only. |
- Original College Internship Duration Certificate.
- Original University Passing Certificate with one Xerox copy.
- Original Date of Birth Certificate with One Xerox copy.
- All above Xerox copies should be countersigned by your College Principal.
- Registration Fee Rs. 500/-
- Two passport size photographs with signature of the applicant on the front side
of the photograph.

REQUIREMENTS FOR OUT STATE REGISTRATION :- |
- Qualification Certificate in Original.
- Permanent Registration Certificate of State in Original from which he/she has
obtained Qualification.
- Date of Birth in Original S.S.C. Board certificate, School or College Leaving or
proper Birth certificate.
- College internship Certificate in Original.
- College Bonafide or Character or T.C. or leaving in original.
- Every year Mark lists in Original.
- Five passport size photographs with Signature on one.
- Registration Fees Rs. 7000/- + 300/- For I-card.
- If possible give address of the police station close by your college locality.
- You need to submit two Xerox copies of each of the above mentioned originals
countersigned by your College principal where you have received your medical
education.
- Please submit an affidavit on Stamp Paper of Rs.100/.
- Mention Permanent Residential Address of Maharashtra on the Application Form.
- You are requested to submit residential address proof.
